CILA – Community Living Services
Lead QIDP – This position is responsible for ensuring resident health, financial, and recreational needs are met. Assists with the new resident intake process, the comprehensive development and implementation of the Person Centered Plan and Implementation strategies, referrals for alternative or augmentative services, coordination of the discharge process, and assists as needed with resident medical appointments. Provides resident outings. Maintains ICILA caseload in addition to assigned residents of CILA group homes.
Qualifications: Must be QIDP certified with QIDP and supervisory experience. Must have a valid IL Driver’s License and ability to be insured under Oak-Leyden’s vehicle insurance.
