Residents in Oak-Leyden’s 12 family-style homes throughout Western Cook County receive 24-hour support and care that promotes their personal autonomy and physical well-being.
When you become a Home Champion, you support the daily needs and goals of adults with developmental disabilities living independently in the community. Your donations go toward needed exterior and interior maintenance and repairs, keeping Oak-Leyden residents safe, warm, happy, and secure.

$1,000 – $2,999:
Recognitions from $250-$999 donor level
Website listing as a Home Champion
$3,000 – $7,499:
Recognitions from $250-$999 and $1,000-$2,999 donor levels
Name on plaque on Donor Wall (at future date to be determined)
$7,500 and above:
Recognitions from all other donor levels
